Contents of regular birth certificates
A typical Malay birth certificate contains your first, middle, and last name, maiden name (if you married and changed your last name), sex (gender – male or female, or boy or girl), your father’s and mother’s name, date of birth, place of birth, name of issuing agency and place of registration, name and title of signing officer.
Who issues birth certificates in Malaysia?
Under the Birth and Death Registration Act, 1957, registrars of the Government of Malaysia are authorized to issue birth certificates. Birth certificates are also issued by medical centers in Kuala Lumpur, Kelantan, Selangar and other states and cities in federal territories.

You need “certified" English translation of your birth certificate in hard copy
USCIS, NVC, U.S. Passport Office, and many other public and private organizations require “certified translation” of your birth certificate by a third-party, independent translation agency in original format. They do not accept photocopies or electronic copies.
